PEG Laxatives (Miralax, Movicol, RestoraLAX) and Gut Health
Polyethylene glycol 3350, usually just called PEG, is one of the most widely used laxatives in the world.
You've likely seen it under a different name depending on where you live:
Miralax and GlycoLax in the US,
RestoraLAX and Lax-A-Day in Canada,
Movicol, Laxido, and CosmoCol in the UK,
and Movicol or Osmolax in Australia.
Higher-dose versions used for colonoscopy prep go by names like MoviPrep and Klean-Prep.
Despite the different labels, it's the same molecule doing the same job: pulling water into your stool to make it easier to pass.
PEG is often marketed and prescribed as if it were harmless to the gut: a laxative that just adds water to stool and passes through unchanged.
The research tells a different story. Studies using more sensitive molecular methods keep finding that PEG changes gut bacteria. Not just which bacteria are there, but also what they're doing.
Long-term human data is still limited, which is worth noting. But "we don't have much data" is not the same as "we know it's safe." The studies we do have point toward disruption of the microbiome, rather than neutrality.
Let's look at what the evidence actually shows, including where it falls short.
What Is PEG and How Does It Work?
PEG is a synthetic polymer. In medicine, it's used as an osmotic laxative, meaning it works by drawing water into the intestine rather than stimulating the bowel to contract.
Unlike some other osmotic laxatives, PEG isn't fermented by gut bacteria. It passes through the digestive tract largely intact, holding onto water as it goes. That water softens the stool and increases its volume, which makes bowel movements easier and more frequent.
This mechanism is why PEG works differently from stimulant laxatives (like senna or bisacodyl), which trigger muscle contractions in the colon instead of changing stool consistency. It has the added benefit of not causing any dependency.
Low-Dose vs. High-Dose PEG
Most of the research and headlines around PEG blur together two very different use cases, and the distinction matters for anyone deciding how concerned to be.
Low-dose PEG (the Miralax, RestoraLAX, or Movicol you'd take daily for constipation) is a small amount, typically 17 grams, taken regularly.
High-dose PEG (the MoviPrep or Klean-Prep you drink before a colonoscopy) delivers a much larger volume over a few hours specifically to flush the entire colon.
These two doses appear to affect your gut microbiome very differently, which is worth understanding before you assume all PEG use carries the same risk.
Does PEG Affect the Gut Microbiome?
This question usually gets the following answer: "PEG is inert, it doesn't touch your microbiome." That claim traces back mainly to a single trial. Except it doesn't hold up once you look at the fuller body of research. You can skip over this section if you do not want to know the knitty gritty of the different studies.
First, the reassuring study:
A 2004 trial gave adults with chronic constipation either PEG or lactulose for four weeks. Researchers tracked their fecal bacteria using culture-based methods: bacteria had to grow in a lab dish to be counted. By that method, lactulose changed the bacterial populations, while PEG didn't (Bouhnik et al., Alimentary Pharmacology & Therapeutics, 2004).
This one study is the main basis for most "PEG doesn't affect your microbiome" claims online. But culture-based methods only catch bacteria that grow well outside the body. That's a small fraction of what actually lives in your gut. This method also wasn't sensitive enough to catch shifts in overall diversity.
What more sensitive testing methods actually find:
Newer studies use 16S rRNA gene sequencing instead. This method reads bacterial DNA directly, rather than waiting to see what grows in a dish. Under this method, PEG's effects become visible.
One study gave healthy adults PEG to induce osmotic diarrhea, then sequenced their gut bacteria. It found a significant drop in phylotype richness, a core measure of bacterial diversity. It also found a shift in the ratio of Firmicutes to Bacteroidetes, the two dominant bacterial groups in the human gut, along with a relative rise in Proteobacteria: a shift often linked to dysbiosis in other GI research (Ott et al., PLOS ONE, 2013).
PEG changes what bacteria do:
A rat study found that PEG lowered microbial bile acid metabolism and lipid metabolism in the gut. The bacteria that survived weren't just present in different proportions; they were functioning differently (American Journal of Physiology-GI and Liver Physiology, 2012). Mouse studies on PEG 3350 specifically have found similar shifts in diversity after treatment.
Bowel-prep doses hit harder:
The large volumes used before a colonoscopy work like a full flush. A 2025 study found that PEG-based bowel prep rapidly disrupted the gut lining in animal models. It also temporarily raised the risk of pathogens taking hold, and let bacteria travel to other organs (Cell Reports Medicine, 2025). Other research has found bacterial abundance can drop more than 30-fold right after a bowel-prep dose. Recovery usually takes around two weeks.
Where this leaves us:
PEG isn't proven totally safe for your microbiome. It also isn't proven harmful.
Large, long-term human studies on daily low-dose PEG still don't exist. But given what the more sensitive studies already show, I don't think it is fair to default to "it’s probably fine."
Is PEG Safe for Long-Term Use in Adults?
PEG isn't absorbed into the bloodstream in meaningful amounts.
It also doesn't cause physical dependency like some stimulant laxatives do. A meta-analysis comparing PEG formulations found consistent efficacy and a good safety profile across studies (BMC Gastroenterology, 2016). But that study measured tolerability and systemic side effects, not microbiome and gut-health impact.
Given the evidence above, I'd treat daily PEG use more as a tool with real tradeoffs.
A few practical considerations:
It treats the symptom, not the cause.
If you need PEG every day for months, ask what's driving the underlying constipation (see: Constipation and SIBO)The evidence on the microbiome impact points to a disruptive mechanism
I'd rather someone use a different osmotic laxative, or use PEG short-term, while addressing the root cause than lean on it indefinitely.Electrolyte balance has to be looked at.
Osmotic laxatives will lead to more mineral loss. If you're restricting fluids, have kidney issues, or take other medications that affect electrolytes, caution is advised.PEG doesn't fix motility itself.
It makes stool easier to pass. It doesn't restore normal gut movement if an underlying motility issue, like impaired MMC activity, is the real driver.
Common Side Effects of PEG
Most people tolerate PEG well. The most commonly reported side effects are mild and dose-related:
Bloating
Gas
Abdominal cramping
Nausea
Diarrhea, if the dose is too high
These usually resolve by adjusting the dose downward. If you experience severe abdominal pain, vomiting, or no bowel movement despite use, that warrants medical attention rather than a dose adjustment.
PEG for Kids: What Parents Should Know
PEG, especially Miralax, is one of the most commonly used treatments for constipation in toddlers and young children. It works well. Several studies have found it works better than other pediatric laxatives for chronic constipation.
It's also, notably, not FDA-approved for use in children. This is where the safety conversation gets more complicated.
Between 2007 and 2017, the FDA's adverse event reporting system logged over 1,500 adverse symptoms in children who'd taken PEG. Most of those were neurological or psychiatric: anxiety, mood changes, tics (Hussain et al., GastroHep, 2019). The FDA reviewed these reports more than once and concluded the existing label was accurate.
A large 2025 study compared over 645,000 children on Miralax to more than 100,000 on a different constipation treatment. The picture varied by age. Children 6 to 11 were actually less likely to develop new depression, mood swings, or seizures on Miralax. Children 12 to 17 were less likely to develop depression, but more likely to develop new or recurring anxiety, mood swings, and seizures (Epic Research, 2025).
What this means in practice: no study has confirmed a causal link between PEG and neuropsychiatric symptoms in kids.
The largest recent data actually looks reassuring for younger children. But the signal in older kids and teens hasn't been fully explained, and PEG remains an off-label use in pediatrics. If your child needs PEG for more than a week or two, do it under a pediatrician's guidance rather than managing it alone, and flag any new behavioral or mood changes instead of assuming they're unrelated.
Practical Tips for Parents Considering PEG for Kids
Favor short-term use. PEG is best used for a defined course, not indefinitely, unless a doctor is actively managing longer-term use
Watch for changes, not just in bowel habits but in mood or behavior, and mention anything new to your pediatrician.
Support the gut with diet, not just the laxative: fiber, fluids, and a varied diet all help build resilient bowel habits alongside any medication (see: How to Increase Butyrate Naturally)
Should You Be Concerned About PEG and Your Gut?
PEG has a strong safety record when it comes to systemic side effects and short-term tolerability, and it works well for relieving constipation. But "safe" in that narrow sense has gotten conflated with "harmless to your gut microbiome," and the more sensitive research doesn't support that leap.
The molecular-level human and animal studies we have consistently show PEG shifting bacterial diversity and function, not leaving it untouched, and that effect increases sharply at the higher doses used for bowel prep.
The same caution applies to children, as the medication is not FDA-approved for pediatric use and has been associated with potential neuropsychiatric side effects.
My view: PEG is a useful short-term tool, not something to lean on indefinitely.
